Patient Access Health Insurance Medicaid Medicare Liability Workers Comp Financial Assistance If you received a letter from us, we would like to help you resolve your outstanding bill. We can bill or re-bill as well as follow up on unpaid claims. Please read the following to see which option may apply to your particular situation. Click on the appropriate option located below for additional information. Please go to HEALTH INSURANCE if you had health insurance, PPO, HMO or other managed care at the time your medical bills were incurred. Please check your records to confirm that your health insurance has not already paid their portion of your outstanding bills, and that the remaining amount is not a patient portion, deductible amount or co-pay. If you were injured in an automobile accident, or if your medical treatment was the result of injuries suffered as a result of another’s negligence, you may be able to obtain payment of your outstanding medical bills from another party. As you are aware, you are primarily responsible for payment of your outstanding medical bill. Should we not be successful in obtaining payment from another party, you will be expected to pay the balance due the provider of medical services. You must act quickly and cooperate fully with our office to obtain reimbursement from another responsible party. Please go to LIABILITY and complete the on-line questionnaire. If you were injured at work, or as a result of a work-related activity, you may be entitled to have your medical bills paid through the provisions of the WORKER'S COMPENSATION LAW. Again, you are primarily responsible for payment of your outstanding medical bill. Should we not be successful in obtaining payment from your employer, you will be expected to pay the balance due the provider of medical services. You must act promptly and cooperate fully in order to obtain reimbursement from your employer. Please go to MEDICARE if you are currently receiving Medicare benefits. Please go to MEDICAID if you are currently eligible to receive coverage for medical services(or if you believe that you may be eligible to receive benefits) from the Illinois Department of Public Aid. If you are willing to apply for financial assistance you can fill out our FINANCIAL ASSISTANCE APPLICATION form. If you are not eligible for one of the programs indicated above, our office can assist you in repaying the outstanding balance due. Please go to MAKE PAYMENT for further information. For your convenience we are able to accept payment by credit card or by checks automatically deducted from your account. Please understand that this is an attempt to collect a debt by a debt collector, and that any information we obtain will be used for the purpose of collecting this debt.
